Below are links to individual forms needed by 2017-2018 Henry Clay Band students. Please fill these out and turn them in as soon as possible but no later than the first day of band camp for marchers or the first day of school for non-marchers. Please note that the Medical Release Form is required to be completed and turned in before participation in band camp! Also note that none of the forms are required to be notarized. Corporate Donor Program -support the band by becoming or recruiting a sponsor
Students needing Financial Assistance must be approved for Fee Waiver through FCPS. Parents must contact Mr. Kite or Mr. Bayerle at email@example.com or firstname.lastname@example.org by May 20, 2017 to verify and provide the proper signed form. Parents and students must understand that they are expected to help the HCBB absorb the cost through fundraising to remain in good standing in the program.
